Health Information Clerk- part time

Job Description
This position manages and scans records. Locates, logs, retrieves, tracks, and delivers health information as assigned. This includes but is not limited to providing answers to patients, providers and clinic staff regarding patient information.
Responsibilities
Pull EMR requests for requested medical records per guidelines. Deliver, transport, sort, and file returned records. Scan all medical records as applicable and accurately. Scan all correspondence in the appropriate place within the electronic medical record. Destroy outdated records following established procedures for retention and destruction. Answer telephones while taking accurate messages and giving routine information in accordance with established procedures. Release information or transfer medical record information per UFM policies and according to HIPAA regulation. Maintain confidentiality of all employee, patient, and other privileged information regarding UFM business activities and according to HIPAA standards. Maintain the highest standards of ethics and integrity in the performance of job duties and responsibilities. Other duties as assigned.
Qualifications
High School Graduate or GED required 3-5 years experience in Health Information required

CMA/RMA or LPN

Job Description
United Family Medicine has a unique opportunity for a CMA/RMA or LPN. Rooming patients for variety of providers. 3-5 years Experience working in a clinic required. Provide high quality and efficient direct patient care services including rooming patients, completing vitals, administrative duties, stocking and cleaning and customer service. All positions required to work 1 evening a week until 8:00pm and an occasional Saturday from 8:00am to 1:00pm.
Responsibilities
Rooming patients including taking vital signs, history, and preparing patient and equipment to be seen by the provider. Performs routine procedures including EKGs, audiograms, vision and hearing screenings, ear irrigations, TB skin tests, inspirometry and assisting with minor surgery and other procedures. Administers medications and vaccinations per provider order.
Qualifications
Certified or Registered Medical Assistant or Licensed Practical Nurse with 5 plus years experience in ambulatory care. LPN licensure required. CMA or RMA certificate needed or must receive within 6 months of hire. Previous clinical experience at UFPHC strongly desired. Spanish Speaking Candidates encouraged to apply.

RN - Triage (internal applicants only)

Job Description
Qualified RN to work telephone triage in an ambulatory setting. Triage patients and provide follow up calls as needed, schedule appointments using the triage documentation system in Excellian and take messages for providers as needed. This position must be able to work quickly with a large volume of calls while showing exemplary customer service in a dynamic and collegial working environment.
Responsibilities
Telephone triage- answer patient questions as needed, schedule appointments, follow up, intake paperwork and assistance with provider messages.
Qualifications
RN with current MN licensure and five years clinical triage or related nursing experience. At least 3 years experience in an ambulatory care setting and triage experience required. Must be flexible and organized and able to demonstrate sound nursing judgment and critical thinking skills. BLS certification Required Experience using electronic medical records a plus.
Position Details
Set hours Monday - Friday from are 8:45am to 5:15pm Additional hours during the week covering triage could be in as early as 8:00am to cover phone lines.

Dental Assistant

Job Description
Perform chair-side assisting of dentist, clinical support services and assigned related work under the direct supervision of the Dental Director.
Responsibilities
Essential Functions: Assist dentist chair side during patient procedures within the scope of general dentistry using four handed delivery techniques. Proficient in taking clinically diagnostic digital X-rays, and maintain equipment as outlined in safety manual and radiation safety manual. Sterilize, disinfect, and maintain instruments and operations according to OSHA and clinic protocols. Perform daily start-up and close-down procedures in operatories and lab. Set-up and clean-up operatories, materials, and equipment needed for dental procedures Take patient blood pressures and record in chart. Accurately chart results of oral exam as instructed by provider. Perform child prophylaxis, fluoride treatment, and place sealants. Take alginate impressions, pour and trim models, and prepare cases for outside lab. Monitor nitrous oxide administered by provider. Perform oral hygiene instructions. Handle and dispose of infectious and hazardous waste according to clinic protocols. Answer phone, screen calls, confirm appointments, and do recall as needed. Order and inventory supplies, and perform inventory audits as scheduled. Attend staff meetings and contribute to planning and workflow of dental clinic. Have respect, courtesy and great rapport with patients and staff.
Qualifications
Qualifications: High school diploma or GED equivalent. Certificate of graduation from an accredited dental assisting school. Maintain a current active registration in dental assisting in the state of Minnesota. Previous work experience desirable. Clinical fluency in Spanish preferred. Expanded functions preferred. Desire to work within a dental public health center.

Bilingual Licensed Social Worker, MSW, LICSW (Spanish)

Job Description
Social Worker works closely with patients to provide access to resources and therapy, social rehabilitation, crisis intervention and outreach services. Assure care of all ages is optimized with a special emphasis on the social service needs of patients. Special focus will be in the moment crisis resolution with patients. May also offer individual sessions to patients, instruct other mental health staff in therapeutic techniques, provide crisis interventions, arrange for services from referral agencies and help patients ease back into the community after leaving inpatient programs. Patient and community outreach, customer service and specific program initiatives will occur as needed and as time permits.
Responsibilities
Conduct interviews in order to determine the appropriate services to offer patients and their families. Develop care plans that include counseling, support services, treatment methods and referrals. Reviewing care plans as necessary with patients and make changes to the care plans as necessary. Explain treatment plans to patients and their families, maintaining patient records, preparing reports, monitoring progress toward treatment goals and conducting annual reviews of active treatment plans. May also offer individual and group therapy sessions to patients, instruct other mental health staff in therapeutic techniques, provide crisis interventions, arrange for services from referral agencies and help patients ease back into the community after leaving inpatient programs. Improve patient outcomes, and deliver quality care within practice areas as assigned. Ensure all clinical care and care coordination is delivered in a manner that meets or exceeds goals and expectations for clinical outcomes, quality assurance standards, and patient satisfaction. Participates in quality strategies to evaluate compliance with behavioral health standards and Perform an assessment based on the patient?s diagnosis, reason for entry into system and care needs. Plan, perform, delegate and evaluate the delivery of services. Ensure all patient charts and related documentation are maintained current and relevant at all times.
Qualifications
Education MSW degree; LICSW, LISW, or LGSW degree and licensure in the state of MN Must be licensed and able to bill for services. Certifications/Licenses Must maintain supervision practice agreement with a LICSW until the 3000 practice hours have been completed and the test for LICSW has been completed. At least 5 years of experience working directly with patients Spanish Speaking required.
